要約　背景：眼内レンズ（IOL）挿入眼での前囊収縮は，水晶体の上皮細胞が筋線維芽細胞に変化することで生じるとされている。目的：シングルピースIOL挿入眼に強い前囊収縮が生じ，視機能が低下した2症例の報告。症例と所見：症例は67歳女性と73歳女性で，いずれも両眼に白内障手術とアクリル製IOL挿入を受けた。IOLは症例1では右眼に3ピース，左眼にシングルピースであり，症例2では両眼ともシングルピースであった。症例1では術前からZinn小帯が脆弱であり，手術の5か月後に左眼の前囊が強く収縮して前囊窓が閉鎖し，YAGレーザーで切開した。症例2では手術の2か月後に左眼の前囊が収縮し，上方のIOL支持部が瞳孔領に移動して視力が低下し，IOL整復術を必要とした。結論：Zinn小帯が脆弱なIOL挿入眼では前囊収縮の可能性があり，IOLの選択や術式に注意する必要がある。

Abstract.　Purpose：To report 2 eyes that developed contraction of anterior capsule after insertion of single-piece intraocular lens（IOL）. Cases and Findings：Two females,aged 67 and 73 years respectively,received cataract surgery with implantation of acrylic IOL. Case 1 received 3-piece IOL in the right eye and single-piece IOL in the left. Lens zonules were fragile in both eyes. Case 2 received single-piece IOL in both eyes. Case 1 developed contraction of anterior capsule in the left eye 5 months after surgery. Occlusion of capsule opening was relieved by Nd：YAG laser. Case 2 developed contraction of anterior capsule in the left eye with protrusion of upper haptic into the pupillary area 2 months after surgery. Visual acuity was restored by repositioning of IOL. Conclusion：These cases illustrate that anterior capsule may contract in pseudophakic eyes,particularly in those with fragile zonules. This possibility has to be kept in mind in the selection of IOL and surgical approach.
